Volume Management

Volume Management is an important, but also a challenging issue in clincial procedues.

In many treatment scenarios it is crucial to infuse high volumes of fluids within a very short period of time. Ideally those fluids are infused in controlled temperature ranges to avoid body temperature modulation; ideally a feedback mechanism tighly controls the body core temperature and relevant physiological parameters.

tempedy® facilitates even very complex infusion regimes and provides the required physiological feedback control.

(1) Septic patients

Clinical rationale for tempedy:

Precise and rapid application of infusions (flow rates up to 250ml/min) for fluid management, in parallel automatic and body temperature feedback controlled temperature management, e.g. in case body temperature exceeds a predefined threshold all applied fluids will be pre-cooled (down to 4°C)  in order to support a fever reduction strategy.

(2) Inadvertent perioperative hypothermia

Background:

Inadvertent perioperative hypothermia (PT) is defined as decrease of body core temperature during operations below 36 °C (NICE 2008) and is one of the most frequent complications related to anaesthesia.

The incidence of postoperative hypothermia is 25 – 90% of all patients undergoing elective surgery in current reviews – such as the S3 guideline of the AWMF (Arbeitsgemeinschaft der wissenschaftlichen medizinischen Fachgesellschaften: 2014; Torossian 2015).

In summary inadvertent perioperative hypothermia negatively influences the operation results and the postoperative outcome causing prolongation of hospitalization and increased treatment costs (NICE 2008, AWMF 2014).

The aim of all warming measures during operation must be to keep the body core temperature (BCT) above 36°C.

Clinical rationale for tempedy:

Intravenous administration of warm infusion solutions reduced the incidence of intra- and postoperative hypothermia when compared to infusions with room temperature (Bernardo 2001; Andrzejowski 2010; Jeong 2008; Hasankhani 2007; Oshvandi 2014; Goyal 2011; Chung 2012).

The clinical rationale of the perioperative application of tempedy® is to stabilize the patient within the predefined normothermia range over the course of the surgical intervention, while automatically minimizing the patients i.v. solution volume burden with flowrate reduction regulated via the body temperature feedback control.
